Intake coordinator directs the call to the most appropriate service.
Geriatric Assessment Outreach Team (GAOT) Geriatric Assessor or VHA Health and Home Support Assessor completes an in home visit and assessment of physical, social, and spiritual needs.
Action:
- Referral(s) to local agency or community group
- Client receives services and/or
- Network engaged to identify gaps and problem solve
